The description says:
-Made of solid hardwood with high-gloss lacquer finish
-Interior is fully lined in velvet; bottom is covered with felt
-Measures 5.5" x 5.5" x 2.25" with a 4.25" x 4.25" ceramic tile

I found this at http://www.cafepress.com. The top is a ceramic tile and not canvas but the box sounds like what you described. I have never bought from them, but it may be worth checking out. I think (???) you can buy the designs they offer or open an account and use your photos.

to sarah3. I am quite sure that bagsoflove ship internationally...Look at their "sizes and prices" and P&P £1,95 UK, £9 EU, World £15 , I think "World £15" refers to US as well:)
